Form MCSA-5889 reinstatement walkthrough

Form MCSA-5889 is FMCSA’s Motor Carrier Records Change Form and includes an operating-authority reinstatement request. Use the current form and instructions, identify the existing company accurately and resolve the underlying prerequisites. It does not replace an MCS-150 update for an inactive USDOT record.

Before completing the form

Check the current authority status, the reason for revocation and whether company information matches the agency record. Assemble the business identifiers and required supporting material. Verify the current form version on the FMCSA website before filling it out.

Identification and request sections

Use the legal name and identifiers for the existing carrier, broker or freight forwarder. Read each section’s instructions and complete the portions applicable to your request. An authorized person must review and sign the request; avoid leaving required identification or contact fields incomplete.

Payment and submission

FMCSA’s form lists an $80 reinstatement payment. Follow the current instructions for payment and submission and retain a copy of everything sent. The government payment is separate from a private preparation fee. Do not send card or identity information to an unverified recipient.

After submission

Save the receipt and confirmation number. Respond to requests for missing information and check the resulting operating-authority status. Submission alone does not restore permission to operate, and FMCSA determines whether the reinstatement conditions have been met.

Questions

Does submitting a reinstatement request authorize operations?
