The fixed schedule
Under 49 CFR §390.19T(b), every active USDOT registrant must update its Motor Carrier Identification Report (MCS-150) every 24 months. The schedule is fixed by USDOT digits, not by your fiscal year or anniversary date.
The MONTH is determined by the LAST digit of your USDOT: 1 = January, 2 = February, ... 9 = September, 0 = October. The YEAR is determined by the SECOND-to-last digit: even number = even year (2024, 2026, 2028...), odd number = odd year (2025, 2027, 2029...).
A USDOT ending in ...23 files in March of every odd year. A USDOT ending in ...40 files in October of every even year. The schedule never moves.
The deactivation trap
The most common reason small carriers get deactivated is not understanding that the schedule is fixed. They file MCS-150 once at registration, treat it as a one-time form, and never look at it again. Two years later their USDOT goes inactive without warning.

What deactivation costs
A deactivated USDOT cannot legally operate. Brokers will not tender loads to a deactivated carrier because their cargo policy excludes non-authorized haulers. Fuel-tax (IFTA) and apportioned-plate (IRP) renewals may be held until the USDOT is reactivated.
Civil penalties under 49 USC §14901 can reach $1,584 per day (capped at $15,846 per violation under 49 CFR part 386, Appendix B) for continued operation while deactivated. The penalty is rarely assessed against owner-operators - but the lost revenue from a 7-to-21-day reinstatement window typically runs $3,000–$10,000 in idle truck cost.
How to reactivate
File the overdue MCS-150 plus an updated MCS-150B if you carry hazmat. There is no separate reinstatement fee for an MCS-150-driven deactivation - just file the form. FMCSA typically reactivates within 24 hours of receiving a complete update.
If your authority was deactivated for more reasons (BMC-91 lapsed, BOC-3 lapsed, or you missed multiple cycles), you may need a full reinstatement under OP-1(R) at $80 plus the usual MC + BOC-3 + BMC-91 cure work.
