Who needs to file MCS-150?
Every entity with an active USDOT number must file MCS-150 every two years under 49 CFR §390.19T. The schedule is fixed: the month is determined by the last digit of your USDOT, and the year is determined by the second-to-last digit (even or odd). Hazmat carriers also file MCS-150B.

Why does it matter?
The biennial update is FMCSA's way of confirming carriers are still operating and keeping current power-unit / driver counts. It also supports CSA scoring - without an up-to-date PU/driver count, the SMS percentile math gets distorted.
MCS-150 is required even if nothing about your operation changed. Filing the same information again is the affirmative act FMCSA wants. A missed update can lead to civil penalties and deactivation of the USDOT number under 49 CFR 390.19T(b)(4), even when “nothing changed.”
