Who needs this template

Every motor carrier that employs a CDL driver. The DQF is the single biggest documentary check in a new-entrant safety audit and a routine compliance review - FMCSA investigators literally sit at the table with a stack of DQFs and walk through this exact list.

Why we built this

About 60% of new-entrant audits we review reveal at least one DQF gap. It's rarely a missing application - it's usually a stale MVR, a missing pre-employment Clearinghouse query, or an annual violations disclosure that fell off after year one. Carriers who assemble the file at hire and never look at it again drift out of compliance silently.

This checklist is sequenced in the order auditors actually verify items. If you can hand an investigator a folder where each tab matches a row on this list, you'll pass on the documentation side. We left blanks for dates and reviewer initials - keep this as a cover sheet on top of every driver's DQF, sign and date each row at hire and at every annual review, and you have a paper trail that shows ongoing compliance instead of a one-time scramble.

The list maps each row to the exact CFR section in 49 CFR Part 391 (and Part 382 for Clearinghouse items). When an investigator asks “where's your authority for that?” you can point to the same section in the printout.
